Digitising Patient Records for Complete Traceability

Children’s NHS services relied on paper-based and fragmented digital processes to record patient interactions, assessments, and clinical notes. Front-line teams struggled with limited access to information, inconsistent record-keeping, and no reliable way to ensure traceability across a patient’s journey. These manual workflows created delays, increased administrative burden, and introduced risks around data accuracy, audit readiness, and continuity of care. The service needed a secure, mobile-friendly digital records system that clinicians could use easily in real-world environments.

We developed a secure, low-code electronic patient records platform designed specifically for community-based clinical teams. The system enables staff to capture, update, and review patient information directly at the point of care, using any mobile device. Role-based permissions, structured data forms, and controlled workflows standardise the way records are created and managed, ensuring accuracy and compliance.

A fully auditable data model underpins the platform, providing 100% traceability of updates across all patient interactions. The interface was tailored to clinical workflows to minimise documentation effort, while optimising usability for busy front-line staff. Integration pathways were also established to support future connectivity with wider NHS systems and reporting requirements.

The new digital platform has eliminated paper records, reduced admin burden, and given clinicians immediate access to accurate patient information wherever they work. Traceability is now complete and automated, supporting safeguarding, audit, and clinical governance requirements.
